A self-reported investigation concerning staff and child interaction was initiated on 4/28/2023, from a report received by the center on 4/28/2023. The investigation was conducted on 5/3/2023 in the afternoon between 12:30 pm and 3:15 pm and completed on 5/8/2023. The investigation included interviews with the management team and staff.
