An unannounced monitoring inspection was conducted today.
There were 16 preschool children (ages 3-5years) present with two teachers.
The children were having group time, eating lunch, and playing in the gym.
Two child records were reviewed.

Standard #: 8VAC20-780-270-A Description: Based on a tour of the playground the facility failed to ensure that all equipment is in a safe condition.
Evidence:
1. On the climber, there is a gap of 1/8 to 1/4 inch between the deck and the slides. This presents an entanglement hazard.Plan of Correction: The climber has been fixed so no entanglement hazard is present.
