Comments:
An unannounced monitoring inspection was conducted with the director on April 24, 2023 beginning at 12:51pm and ending at 2:33pm. There were 29 children, ages 6 months through 5 years old, in the care of 4 direct care staff. The children were observed laying on resting mats during nap time. Five children?s records were reviewed and in compliance. Three staff records were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. There were no medications to be reviewed. The menu, daily attendance, and emergency drill log were reviewed. The emergency drill log was reviewed and in compliance. The most recent health inspection was dated December 5, 2022. The most recent fire inspection was dated January 24, 2023.

Violations:
Standard #: 8VAC20-780-330-B
Description: Based on observation, the facility has not ensured the resilient surfacing complied with the minimum safety standards. Evidence: The resilient surfacing in the fall zones were less than 1 inch and the dirt ground was exposed.

Plan of Correction: The facility was going to be receiving an order of mulch that would be delivered and installed on the playground to come into compliance.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the facility did not ensure the diapering surface would be used only for diapering or cleaning children. Evidence: There was a box diapers, pack of pull-ups, and 2 bins of children's belongings in the diapering area in the preschool classroom.

Plan of Correction: The director stated that she would speak with the staff about storing items in the diapering area.
