Comments:
An unannounced monitoring inspection was conducted on 5/3/2023. There were 30 children, ages 3 years-5 years and 6 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records and 3 staff records were reviewed. The children were observed during circle time, center time and active time on the playground. The inspector arrived for the inspection at 9:44am and departed the center at 10:39am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 22.1-289.058
Description: Based on observation and discussion with staff, the center failed to ensure each building that was built before 2015 and that houses a child day program that is licensed that serves preschool-age children shall be equipped with at least one carbon monoxide detector.

Evidence: Licensing staff did not observe a carbon monoxide detector during the inspection. The program director stated that ?they had one but not sure what happened to it?.

Plan of Correction: Provider will purchase a carbon monoxide detector.
