Technical Assistance:
Technical assistance was given and licensing standards were reviewed regarding resilient surfacing .

Comments:
An unannounced renewal inspection was conducted on May 1, 2023. The inspector arrived at 10:00 AM and departed at approximately 1:00PM. There were 84 children present and 22 staff members. A sample size of 18 records were reviewed which included 8 children?s records and 10 staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, classroom lessons, free play indoor and outdoor, handwashing, diapering, lunch and naptime. First aid kits and emergency preparedness documents were observed. Transportation vans were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site assistant director at the exit interview.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on record review, it was determined that the center did not ensure to obtain a copy of the results of a criminal history record information check from any state in which the individual has resided in the preceding five years.
Evidence:
Staff #2 (date of hire 04/03/2023) noted on their Sworn Statement dated 04/2/2023 that they lived in two other states outside of Virginia in the preceding five years. There was no background check completed for one of the two states listed for Staff #2.

Standard #: 8VAC20-780-60-A-8
Description: Based on record review and interview, it was determined that the center did not ensure that a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
1. Child #1 has a diagnosed food allergy confirmed by a physician however there was no instructions from a physician the steps to be taken in the event of a suspected or confirmed allergic reaction.
2. Staff #1 confirmed that there was no documentation of the steps to be taken in the event of a suspected or confirmed allergic reaction for Child #1?s allergy.

Plan of Correction: The center responded with the following: The mom has taken it down to a restriction, stating he no longer needs to be classified as an allergy so no packet is needed.

Standard #: 8VAC20-780-270-A
Description: Based on observation and interview, it was determined that the center did not ensure that areas and equipment of the center, inside and outside, be maintained in a safe condition.
Evidence:
1. There is peeling paint to the ledge that is in reach of children in the preschool room.
2. There were two cracked plastic containers on the playground that poses as a pinching hazard to the children in care.
3. There is peeling paint along the entire exterior wall of the toddler playground.
4. The 2 year old classroom had the following:
a. There were staples coming off the wall with sharp edges in reach of children in care which poses an injury hazard;
b. There was a wooden shelf that was upright and children were using as a play boat. It was not sturdy and poses as a tipping and injury hazard to the children in care;
c. In the 2 year old bathroom, there were cleansing wipes in the bathroom on top of the toilet paper holder that had a warning label to keep out of reach of children.
d. The toilets in the 2 year old bathroom had exposed screws/bolts to both sides of the toilet that were rusted. This poses as an injury hazard to children.
5. In the school age classroom boy?s bathroom, there is a 2-3 inch hole on top of the sink in reach of the children in care.
6. Staff #1 confirmed all the concerned areas above regarding the peeling paint, concerns to the 2 year old classroom, and the hole in the school age room.

Plan of Correction: The center responded with the following: Staff #1 will put work orders for the painting and repairs for the center. Staff will remove all staples on the wall that poses as an injury hazard. During the inspection, the unstable bookshelf was moved away from the children and placed in a safe area. The broken containers on the playground were disposed of.
