Comments:
An unannounced monitoring inspection was conducted on 5/4/23 from 10:35am to 3:05pm with the new Director and the Assistant Director. There were 82 children in care, ranging in age from three-months to five-years-old, supervised by 14 staff. The children were observed doing puzzles, singing and having free play, painting, washing hands, reading books, playing on the playground, and eating lunch. Four child records and five staff records were reviewed. The Center has multiple staff with current certification in CPR and First Aid as well as staff trained in Daily Health Observation. Five medications and their corresponding authorization forms were reviewed and the center has four staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 2/27/23 and the most recent Health Inspection on file was dated 12/26/22. Lunch served was macaroni and cheese with ham, peas, bananas, and milk. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 5/15/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on review of four staff records, the center did not obtain the results of a search of the central registry for one staff within 30 days employment. Evidence: The record for Staff #4, with an employment date of 11/15/21, contained documentation of central registry results dated 1/4/22.

Plan of Correction: We will ensure that all background checks are completed prior to employees start dates.

Standard #: 8VAC20-780-140-A
Description: Based on review of five child records, the center did not obtain a physical examination by or under the direction of a physician within 30 days of attendance for one child. Evidence: The record for Child C, with a start date of 10/31/22, did not contain documentation of a physical.

Plan of Correction: Got updated physical and put in child's file.

Standard #: 8VAC20-780-60-A-8
Description: Based on review of five child records, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for Child E contained documentation of a diagnosed allergy to peanuts and cashews. There was no allergy care plan from a physician on record for Child E.

Plan of Correction: We have updated the child's file with the correct allergy plan and will ensure that every child with an allergy has an up to date allergy plan on file.
