Comments:
A renewal inspection was conducted on March 20, 2023 at approximately 11:40 AM to 2:15 PM. There were 29 children present, ranging in age from infant to five years, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, and emergencies and nutrition. Medication is administered and medication, authorization forms and allergy plans were reviewed. A total of six child records, seven staff records and two agent records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.


Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures

Description: Based on a review of records and interview, the center did not ensure to develop written procedures for prevention of shaken baby syndrome or abusive head trauma including coping with crying babies, safe sleep practices and sudden infant death syndrome as required.

Evidence:
1. The written procedures for prevention of shaken baby syndrome or abusive head trauma including coping with crying babies, safe sleep practices and sudden infant death syndrome were not observed.
2. Administration acknowledged that the center had not developed the written procedures.

Description: Based on a review of children's records, the center did not ensure that each record contains all required information.

Evidence:
1. The records for child #1 (enrolled: 8/15/22) and child #3 (enrolled: 3/23/21) did not contain the name, address, and phone number for each custodial parent.
2. The record for child #1 (enrolled: 8/15/22) did not contain the work phone number and place of employment for each custodial parent.
3. The records for child #3 (enrolled: 3/23/21), child #4 (enrolled: 02/06/17), and child #5 (enrolled: 2/03/20) did not have documentation of annual updates.
