Comments:
Today an unannounced Renewal Inspection took place between the hours of 10:45am-12:15pm. There were 71 children with 15 staff within the supervision guidelines. The children were observed eating, drinking, listening to stories, playing outside and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log and emergency procedures were observed during this inspection. Violations:
Standard #: 22.1-289.011-F
Description: Based on observation, documents required by the Superintendent were not posted in a conspicuous place on the licensed premises.

Evidence:
1. The findings of the most recent inspection were not posted in a conspicuous place on the licensed premises at the time of the inspection.

Plan of Correction: We will post the required documents from the previous inspection at all times.

Standard #: 8VAC20-780-60-A
Description: Based on a review of children's records, the center did not maintain a record for each child that contained the required information.

Evidence:
1. Child #4 was missing 1 of 2 required emergency contacts (name, address, and phone number).

Plan of Correction: We will complete the child records.

Standard #: 8VAC20-780-70
Description: Based on a review of staff records, all staff did not have the required records kept on file.

Evidence:
1. Staff #2 (doh: 1/9/2023) did not have 1 of 2 required references of character and reputation prior to employment.

Plan of Correction: We will get references and make sure they are done prior to the employee being hired.

Standard #: 8VAC20-780-240-E
Description: Based on a review of staff records, 1 out of 2 staff did not complete orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care within 30 days of the first day of employment.

Evidence:
1. Staff #1 (doh: 2/1/2023) did not have documentation of completed first aid/CPR orientation training at the time of the inspection.

Plan of Correction: We will make sure all staff have FA/CPR within the allotted timeframe.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation, the Preschool classroom did not maintain the required 1:10 (staff: child) ratio at all times.

Evidence:
1. Preschool 1 and Preschool 3 were together in the Preschool 1 classroom as they were watching a video on the large board in the classroom. There was a total of 25 children with only 2 staff members in the classroom from approximately 11:10am-11:11am. A third staff member returned to the classroom at 11:11am.

Plan of Correction: We will ensure that we maintain ratios at all times.

Standard #: 8VAC20-780-500-A
Description: Based on observation, disposable diapers were not disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff
member's hand nor the soiled diaper touches an exterior surface of the storage
system during disposal.

Evidence:
1. The step trash can in the Infants B classroom was not working properly and staff could not open the lid without touching the exterior surface of the trash can when disposing of diapers.

Plan of Correction: We will replace the trash can.

Standard #: 8VAC20-780-560-M
Description: Based on observation, children were allowed to eat and drink while walking around.

Evidence:
1. In the Toddler A classroom, 2 children were observed walking around and eating donuts given to them by their teacher.
2. In the Toddler A classroom, 1 child was observed walking around and drinking from a sippy cup.

Plan of Correction: We will make sure that children are sitting when eating and drinking.
