Comments:
An unannounced monitoring inspection was conducted under the supervision of the Licensing Administrator on May 9, 2023, from 9:10 a.m. to 12:00 p.m. There were 67 children present, ranging in ages from six weeks to five years old with 17 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of number seven child records and 13 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-B
Description: Based on review of records and interview, the center failed to ensure that the sworn disclosure was completed before the first day of employment.
Evidence:
1. Staff #11?s first day of employment was 04/11/2023. The sworn disclosure on file was dated 04/17/2023.
2. Staff #14 confirmed that the sworn disclosure was dated after Staff #11?s first day of employment.

Plan of Correction: Staff will sign their sworn statements on or before their first day.

Standard #: 8VAC20-780-130-E
Description: Based on review of documentation and interview, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. The record for Child #2 (DOB 10/24/2018) indicated the first date of attendance at the center was 01/17/2019 and contained only two immunization records, dated 12/19/2018 and 07/23/2019.
2. The record for Child #6 (DOB 09/13/2019) indicated the first date of attendance at the center was 12/09/2019 and contained only three immunization records, dated 11/26/2019, 09/16/2020 and 04/26/2021.
3. The record for Child #7 (DOB 03/17/2021) indicated the first date of attendance at the center was 06/16/2021 and contained only two immunization records, dated 06/10/2021 and 11/02/2021.
4. Interview with Staff #14 confirmed additional immunization records were not obtained.

Plan of Correction: Spreadsheet is being used to keep track of upcoming immunization updates.

Standard #: 8VAC20-780-140-B
Description: Based on document review and interview, the center failed to ensure that a physical examination was within three months prior to attendance for a child aged seven months through 18 months.
Evidence:
1. The record for Child #4 indicated the child?s first date of attendance was 02/02/2023. The physical examination for Child #4 was dated 10/17/2022.
2. Interview with Staff #14 confirmed this was the only physical examination in the record.

Plan of Correction: Physicals will be obtained within the correct time frame.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation, the center failed to ensure that in areas used by children of preschool age or younger, an electrical outlet had protective covers.
Evidence:
1. In the Infant Care A room, the top receptacle of an outlet located on the left wall did not have a protective cover. This outlet was accessible from a crib in which child #8 was sleeping.

Plan of Correction: Plug covers will be replacing after using the outlet.

Standard #: 8VAC20-780-430-B
Description: Based on observation and interview, the center failed to provide an adequate supply of materials for each age group.
Evidence:
1. In the toddler classroom, all four shelves were turned around and against the wall, making all of the materials and toys, except for the soft climbers, inaccessible to the eight children in care. Staff #18 confirmed that the shelves were turned around and that the children did not have access to the materials.
2. In the 2?s classroom, seven of the eight shelves were turned around and against the wall, making the materials inaccessible to the children. There were 18 children in the room and less than 10 toy vehicles available for the children to play with. No other toys were available for the children to play with.
3. In the preschool 1 classroom, five of the seven shelves were turned around and against the wall making the materials in accessible to the children. There were eight children in the room.

Plan of Correction: Teachers are aware that shelves must stay open for the children.

Description: Based on observation and interview, the center failed to ensure that cribs were identified for use by a specific child.
Evidence:
1. On 05/08/2023, in Infant Care A, Staff #15 was observed getting ready to place Child #9 in a crib labeled with Child #9?s name when Staff #16 told her Child #9 had been sleeping in the crib beside that one, which was labeled with Child #10?s name. Staff #15 then placed Child #9 on a floor mat.
2. On 05/08/2023, in Infant Care A, Child #8 was observed sleeping in a crib labeled with Child #8?s name and with the name of Child #9, both of whom were in attendance at that time.
3. Interview with Staff #17 revealed that Child #10 had been sleeping in the crib labeled with Child #11?s name.

Plan of Correction: All cribs have been labeled with the appropriate names.
