An unannounced complaint inspection was conducted on 5/1/23 to investigate an allegation regarding the physical plant - cleanliness, and programs - behavioral guidance. Based on evidence obtained through observation and interviews, the complaint was determined to be Not Valid. There were violations (not complaint-related) issued as a result of this inspection. An exit interview was conducted on 5/17/2023.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Wednesday, 5/24/2023. Standard #: 8VAC20-780-320-B Complaint related: No Description: Based on observation, the facility failed to ensure that all restrooms were equipped as required. Evidence: Not all toilets in the bathrooms utilized by the children in the preschool, pre-k and school age classes contained toilet paper that was within reach of children. One of the toilets in each of these bathrooms did not contain any toilet paper. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-500-B Complaint related: No Description: Based on observation, the facility failed to follow all diapering procedures, as required, including using a nonabsorbent surface for changing diapers. Evidence: Staff were observed changing children standing up in the bathroom in the two's classroom, and not using a nonabsorbent surface (the changing table.)
