Comments:
A monitoring inspection was conducted on May 11, 2023 from approximately 1:15 PM to 4:30 PM. There were 32 children present, ranging in ages from infants to five years, with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Description: Based on record review and interview, the center did not have documentation of an updated Central Registry finding every five years from the date of the most recent Central Registry finding.

Evidence:
1. The record for staff #4, ( CR: 7/21/17) did not have documentation of an updated Central Registry.
2. The record for staff # 4. (SS: 7/03/17) had documentation of an updated Sworn Statement on 3/08/23. An updated Sworn Statement was required by 7/03/22.

Description: Based on record review, the center failed to ensure that program leaders shall meet all the required qualifications.

Evidence:
1. The record for staff #2 and staff #6 did not have documentation to demonstrate how staff #2 and staff #3 meets program leader qualifications.
2. A member of management confirmed that staff #2 and staff #3 did not have documentation on file to demonstrate program leader qualifications.

Description: Based on observation, the center did not ensure that staff washed their hands with soap and running water after a diaper change.

Evidence:
In classroom #1, a staff member was observed completing a diaper change. After the staff member was finished, she did not wash her hands with soap and running water.

Description: Based on a review of the facility's injury reports, all required information was not documented.

Evidence:
