An unannounced monitoring inspection was conducted today with the Administrative staff from 12:10 pm to 2:00pm. Thirty five children were present with five staff providing supervision. The toddlers were observed either on their cots for naptime or finishing up lunch. The older children were outside eating lunch on the picnic tables. The areas of non compliance are outlined on the violation notice.

Description: Based on a review of the allergy/sensitivity/dietary restriction list and interview with staff, it was determined that the center is not maintaining a current list of those children who have allergies, sensitivities or dietary restrictions, nor was the list dated.
1. The allergy/sensitivity/dietary restriction list was not dated.
2. Documentation for child A shows that the child has an egg allergy however the allergy list has it as a sensitivity.
3. Documentation for child B shows that the child has an allergy to milk, eggs, peanuts, avocado and melon however the allergy lists has these items as sensitivities.
4. Staff reported that child D has a dairy sensitivity however that information was not on the allergy/sensitivity/diet restriction list.

Plan of Correction: We will revamp our allergy, sensitivity and dietary restriction list to make sure it is accurate and dated.
