Technical Assistance:
Technical assistance offered related to: food from home, OTC products.

Comments:
A renewal inspection was conducted on 5/15/23 from 11:15 am to 12:45 pm. There were 51 children present with 11 staff supervising. The children were observed engaged in various learning and play activities including but not limited to: outdoor play, circle time, puzzle and peg boards, and eating lunch. The lunch menu included macaroni and cheese, mixed salad and milk. The Pre-Kindergarten was observed preparing for their graduation ceremony. 5 staff records, 3 children's records, over the counter ointments and the emergency drill records were reviewed for compliance. The playground was inspected.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on records review, the center did not obtain a tuberculosis (TB) screening for a new staff within 30 days prior to the first day of employment.
Evidence: Staff # 1's first day of employment was 4/17/23. Staff # 1's TB screening was dated 12/28/21.

Plan of Correction: Moving on all TB's for new hires will be obtained according to the timeline in the standards.

Standard #: 8VAC20-780-80-A
Description: Based on records review, the center did not maintain a complete written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: The written attendance for children in care did not document the departure of each child.

Plan of Correction: Attendance that includes the arrivals and departures of all children will be maintained.

Standard #: 8VAC20-780-270-A
Description: Based on observation, equipment outside the center was not maintained in a safe condition.
Evidence: The metal see saw on the playground has a pole in the center that is broken and rusty. The hole creates an injury hazard to the children.

Plan of Correction: The equipment will either be removed or repaired. In the meantime, duct tape has been applied to keep the children from touching it.

Standard #: 8VAC20-780-520-B
Description: Based on observations, the full requirements for using sunscreens was not met.
Evidence: Child # 1's sunscreen was not labeled; and the center did not obtain written authorization for the sunscreen.

Plan of Correction: Written authorizations will be obtained and product will be labeled.

Standard #: 8VAC20-780-520-C
Description: Based on observation, requirements were not met for the use of diaper ointments.
Evidence: Child # 2's diaper cream was not labeled with the name; and the center did not obtain written authorization for the cream.

Plan of Correction: Written authorizations will be obtained and product will be labeled.
