Comments:
A renewal inspection was conducted on May 11, 2023 from 10:00 A.M.-12:45 P.M. There were 93 children present, ranging in ages from 2 years to six years of age, with 18 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of nine child records,nine staff records, and five agent background checks were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review of staff records, the center failed to obtain a completed out-of-state-central registry in the required timeframe.

Evidence:
1. Staff #1 disclosed on the sworn disclosure statement dated 01/31/2023 that they had lived in one other state within the past five years.
2. There was not a completed out-of-state central registry finding on file. The request was mailed on 02/14/2023, but there was no follow up completed regarding the status of the out- of-state central registry finding.
3. Staff #8 verified that follow up had not been completed regarding the background check.

Plan of Correction: We contacted the Center in North Carolina and have now been told that this needs to be completed online. We are currently working on the online submittal. Evidence submitted to licensing inspector.

Standard #: 8VAC20-780-280-B
Description: REPEAT:
Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location.

Evidence:
1. In Classroom #5 the cabinet that houses the cleaning supplies was not locked. In the cabinet there was table solution, Lysol Cleaner, Sure Scents Hawaiian Air Freshener, Disinfecting Wipes and Lysol Disinfecting Spray.
2. In the cabinet next to the cleaning supplies there was a supply cabinet with 2 cans of Shield Shaving Cream and one can of Assure Shaving Cream.
3. Manufacutures labels stated "keep out of reach of children" and "caution"

Plan of Correction: A training has been conducted with all staff to ensure that they understand that all hazardous substances must be locked away safely from children. Attached please find the record of their signatures following the retraining. Retraining completed 5/19. The administrators will spot check daily (3 rooms) to ensure all hazardous substances are locked away (attached find the record of the last 3 days). Spot checks began 5/17 and will continue until 6/7 when school ends for the current school year.

Standard #: 8VAC20-780-520-C
Description: Based on observation, the center failed to ensure that diaper ointment was inaccessible to children.

Evidence:
1. In Classroom #6 there was a tube of Desitin and Triple Paste diaper ointment in a container in cubbie holes located on the outside of the diaper changing table that faces the classroom.
2. The cubbies were accessible to the children.
3. Staff #7 verified that the diaper ointment was accessible to children.

Plan of Correction: This has been corrected by storing all diaper cream in the locked cupboard
