A subsidy health and safety inspection (SHSI) was conducted on 05/15/2023 from approximately 9:30am to 12:20 pm. There were 38 children present with 7 staff. A licensing inspection was conducted on the same date as the subsidy supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the provider.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Standard #: 8VAC20-790-600-F Description: Based on staff records review and interview, the vendor did not ensure that each staff complete annually the Virginia Annual Health and Safety Update Course sponsored by the Department.
Evidence:
1. The records for Staff #1 (date of hire: 10/26/2015), Staff #2 (date of hire: 08/30/2017), and Staff #4 (date of hire: 10/06/2014) contained documentation of the Virginia Annual Health and Safety Update Course dated 1/17/2020.
