Comments:
An unannounced monitoring inspection was conducted from 1:25 PM to 2:50 PM. At the time of entrance, 12 children were in care with 3 staff members present. The sample size consisted of 5 children?s records, 3 new staff records, interview with 3 staff and the background checks for 3 individuals related to the renewal application. Children were observed during rest period, preparing for the end of the day. Areas of non-compliance are identified on the violation notice.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review and staff interview the licensee did not ensure documentation of the Tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children.

Evidence: Center Director confirmed that the following staff had not obtained TB screenings 30 days prior to the first day of employment.
1.Staff #2 first date of employment was 1-13-23 and TB screening 2-23-23
2.Staff #3 first date of employment was 5-15-23 and TB screening 12-8-22

Plan of Correction: Each employee will be informed to obtain a TB screening no more than 30 days prior to their first day of employment.

Standard #: 8VAC20-780-60-A
Description: Based on record review and staff interviews, the licensee did not ensure there was a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: The record for child # 5 has a diagnosed food allergy (peanuts) and the record did not include a written care plan, to include instructions from a physician.

Plan of Correction: Spoke to mom who will provide a written care plan from her sons physician. A written care plan will be written for each child with a diagnosed food allergy and kept on file.
