An unannounced monitoring inspection was conducted on 05/16/2023. There were 130 children, ages 3 months-12 years, and 19 staff members present during the inspection. The inspector observed the following: arrival into the program, afternoon snack, large motor play outside, and diapering. A total of 5 children?s records and 5 staff records, were reviewed. The inspector discussed the following with staff: attendance requirements, playground safety. The inspector arrived at 2:15 PM and departed at 4:30 PM.

Standard #: 22.1-289-036-B-4 Description: REPEAT VIOLATION
Based on review of one board officer?s the center failed to ensure that all out of state background checks were obtained.
Evidence:
Board Officer # 1 took office on 2/28/2022 and lived outside of the state of Virginia in the past five years in a non NFF state (national fingerprint file). The Board Officer is required to have a criminal background from that state. The out of state CPS check was completed on 2/21/2023.Plan of Correction: Not available online. Standard #: 8VAC20-770-60-B Description: Based on review of staff records, the center failed to obtain that all staff had a completed sworn statement.
Evidence:
Staff # 5 had a hire date of 02/14/2023. A completed sworn statement was completed 02/20/2023.Plan of Correction: In the future all Sworn Statements will be completed prior to starting the first day.
Standard #: 8VAC20-780-570-H Description: Based on observation of the infant room, the center failed to ensure Prepared baby food not consumed during that feeding by an infant may be used by that same infant later in the same day, provided that the food is not served out of the baby jar and is dated and stored in the refrigerator; otherwise, it shall be discarded or returned to the parent at the end of the day. Formula or breast milk shall not remain unrefrigerated for more than two hours and may not be reheated.
Evidence:
A half used opened baby food container was in the refrigerator that was dated 5/15/2023. The food need to be discarded or consumed on 5/15.Plan of Correction: The food was discarded and staff will monitor.
