Comments:
A renewal inspection was conducted on 05/22/2023. There were 23 children present and 3 staff. A tour of the facility was conducted, in addition to a record review and interviews with staff. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-70
Description: Based on record review the center did not ensure all required information was kept for each staff person.

Evidence:
Staff #1 and #2 did not have written information that demonstrated they had the experience and education required for their position as program leaders.

Plan of Correction: Both staff have the qualifications for their positions. The documents are electronically saved. The owner will retrieve those documents and send to the inspector and then filed in each staff file.

Standard #: 8VAC20-780-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.

Evidence:
1. There was peeling paint along the walkway into the center and playground.
2. There was a hole in the wood walkway that measured approximately 1 inch in circumference

Plan of Correction: The owners will contact the landlord to correct the peeling paint and holed in the wooden walkway.

Standard #: 8VAC20-780-330-B
Description: Based on observation the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.

Evidence:
On the playground there was no resilient surfacing under 3 pieces of playground equipment with moving parts or climbing apparatus in the fall zones.

Plan of Correction: The owners will have resilient surfacing installed underneath the playground structures the meet the fall zone and material requirements.

Standard #: 8VAC20-780-350-I
Description: Based on observation the center did not ensure all requirements were met during the designated rest period.

Evidence:
1. In the front preschool room there was not a staff within sight and sound at all times in the same space as the resting or sleeping children. T

a. The staff member in the front room left the 11 children unsupervised when she went into the kitchen for supplies.

b. The staff member in the twos room left 4 children unsupervised when she went into the kitchen.

2. Staff in the front preschool room was observed supervising more than one room during rest time. Only one room is permitted.

Plan of Correction: The kitchen is next to the classrooms and staff could hear the children. In the future staff are to be in sight and sound supervision of resting children at all times and only supervising one class.

Standard #: 8VAC20-780-430-C
Description: Based on observation the center did not ensure all play equipment requirements were met.

Evidence:
1. There was a red metal play structure on the playground that had pinch points.
a. There were multiple seats for children to bounce up and down on that were attached by a pole that joined in the middle. The space where the pole and middle met, posed a pinch hazard.
b. The seats attached to the piece of equipment moved up and down creating an additional pinch hazard.

Plan of Correction: The installers assured the owners that the equipment met licensing regulations. The owners will be contacting the playground equipment company to the red play structure removed.
