A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Based on record review the center failed to have one staff member complete the departs annual health and safety course.
Evidence:
1. Five staff records were reviewed.
2. Staff #3 did not have documentation of completing the departments annual health and safety training.
3. Staff #3 last documented training was dated 03/14/2021.
