Technical Assistance:
Technical Assistance was given, and licensing standard were reviewed regarding child records and phone numbers.

Comments:
An unannounced renewal inspection was conducted on May 23, 2023. The inspector arrived at 9:40am and departed at approximately 12:15pm. There were 114 children present and 25 staff members. A sample size of 20 records were reviewed which included 11 children?s records and 9 staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, indoor and outdoor play, arts and crafts, handwashing, and story time. First aid kits and emergency preparedness documents were observed. Medications were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site director at the exit interview.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review and interview, it was determined that the center did not ensure that each child enrolled at the center have a physical examination by or under the direction of a physician: 1. Before the child's attendance; or 2. Within 30 days after the first day of attendance.
Evidence:
1. Child #1 (date of enrollment 09/07/2022) did not have a physical examination by or under the direction of a physician in their record.
2. Staff #1 and Staff #2 confirmed that Child #1 did not have a physical examination by or under the direction of a physician in their record.

Plan of Correction: The center responded with the following: The physical examination record was located and has been placed in the child?s record. The Center will ensure that all children?s records have documentation of a physical examination in their record.

Standard #: 8VAC20-780-260-B
Description: Based on record review and interview, it was determined that the center did not ensure that after the first license, annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for: 1. Water supply; 2. Sewage disposal system; and 3. Food service, if applicable.
Evidence:
1. The last health inspection completed by the City of Chesapeake was on January 27, 2022, with the expiration date of January 27, 2023.
2. Staff #1 confirmed that the last health inspection was completed January 27, 2022, and the expiration date of January 27, 2023.

Plan of Correction: The center responded with the following: The center called the City of Chesapeake during the time of the inspection and scheduled an annual health inspection for May 31 at 10 AM.

Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, it was determined that the center did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the girl?s bathroom, there was a can of disinfectant spray not locked on a shelf that has a warning label, ?Keep out of reach of children.?
2. In the boy?s bathroom, there were a bottle of disinfectant wipes not locked that has a warning label, ?Keep out of reach of children.?
3. Staff confirmed that both bathrooms had cleaning materials that were unlocked in reach of children in care.

Plan of Correction: The center responded with the following: The hazardous substances, Lysol Spray and Lysol wipes, were unlocked but not in the reach of children. The items were on shelves 2 feet above the children?s heads without climbing items near by. Children were, at all times, within sight and sound supervision while in the bathroom. During the course of the inspection, the cleaning materials were removed and locked up. Moving forward, the cleaning materials will continue to be accessible in the children?s bathrooms whoever will be securely locked and out of reach of children.
