An unannounced monitoring inspection was conducted on 5/23/23 from 9:15am to 11:05am with the Director. There were 75 children in care, ranging in age from two-years to five-years-old, supervised by 12 staff. The children were observed playing on the playground, having a class birthday party, practicing writing, washing hands, and playing with tabletop toys. Five child records and two staff records were reviewed. The Center has 16 staff with current certification in CPR and First Aid, and 17 staff trained in Daily Health Observation Training. Four medications and authorization forms were reviewed and the center has six staff current in Medication Administration Training (MAT). Required postings were observed. The attendance and emergency drill log were reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 5/26/23. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 22.1-289.035-B-2 Description: Based on review of two staff records, the center did not obtain documentation of the results of a national fingerprint-based background check prior to employment for a staff member. Evidence: The fingerprints on record for Staff #1, with an employment date of 9/2/22, were dated 9/13/22. Plan of Correction: This was sent in the fall and not returned by California. We continue to work with Staff #1 to obtain CA clearance from the CA Dept of Justice. These two checks will be resubmitted.
Standard #: 22.1-289.035-B-4 Description: Based on review of two staff records, the center did not obtain the results of a criminal history record check or sex offender registry check from all states in which a staff member has resided within the past five years prior to employment, or the results of a central registry check from all states in which a staff member has resided within the past five years within 30 days of employment. Evidence: The record for Staff #1, with an employment date of 9/2/22, contained documentation that the staff member had resided in California within the past five years. The record did not contain documentation of the results of a criminal history check prior to employment or a central registry check within 30 days of employment from California. The sex offender registry check for the staff member was dated 5/23/23. Plan of Correction: Continue to educate staff of the need for checks to be back prior to hire. The fingerprints were received on 9/13/22.
