Comments:
An unannounced inspection was conducted on 05/09/2022 from approximately 9:00 a.m. ? 11:30 a.m. There were 20 children present ranging from infants to 4-year-olds and 4 staff supervising. The inspectors reviewed the areas of non-compliance that resulted in the issuance of the Intensive Plan of Correction (IPOC). The information gathered during this inspection determined continued non-compliance in several areas of the applicable standards. Description: Based on record review, the provider did not ensure staff records are kept for one of three staff.
Evidence:
The staff record for S2, employed 04/12/2023 did not document an emergency contact and information about any health problems that may interfere with fulfilling the job responsibilities.

Description: Based on observation, the provider failed to ensure that resilient surfacing shall comply with minimum safety standards describe in the American Society for Testing and Materials (ASTM).
Evidence:
1. Licensing inspector observed one climbing structure in the playground that had less than the required six inches of resilient surfacing in the fall zone.
2. The playground floor is covered with a mixed surfacing of grass yard fabric and rubber mulch. Mixed resilient surfacing materials without testing and documentation from the manufacturer that supports the combined surface?s resiliency does not comply with minimum safety standards as described in the ASTM standard F1292-99.

Description: Based on observation and interview, provider failed to maintain staff-to-child ratios as required by 8VAC2-780-350.B.1 and B.2.
Evidence:
The infant room and the Busy Bee classrooms were observed to be out of child-to-staff ratio. The staff-to-child ratio in the infant classroom was 1 staff to 6 infants (8 months to 12 months). The required ratio for infants is 1 staff for every 4 infants. The staff-to-child ratio in the Busy Bee classroom was 1 staff to 7 children (16 months to 3 years old). The required ratio, based on the age of the youngest child in the group, is 1 staff to every 5 children. Staff in both rooms acknowledged being out of ratio.
