Comments:
A Renewal inspection was conducted today. A total of 87 children with 20 staff providing supervision was onsite during the inspection. The Toddlers were outside on the playground playing on the equipment and drawing on the fence panels with sidewalk chalk. Three staff were strategically spaced throughout the area to ensure that adequate supervision was provided. The children were riding trikes, drawing and taking turns sliding down the slide where staff were at the base to ensure their safety. All classrooms were preparing for the upcoming graduation celebration. Crafts such as Statue of Liberty crowns were on display and preparation for the sound system for the ceremony was underway. In the Infant class, 3 staff were on the floor with 2 infants actively engaged with them by shaking rattles and other toys. The transition to lunch was observed where children in the Trainers, Two's Preschool 3 and Pre K class washed their hands and were seated at tables for lunch. Lunch today was pasta with meatballs, spinach salad and fresh sliced apples. Ample amounts to offer second helpings which some children requested. Staff were observed sitting with the children as they ate. A total of 7 staff records and 6 children's records were reviewed reviewed and found complete. All medication onsite and topical ointments in the Toddler class were reviewed. The center was found clean and the recordation system organized. This inspection occurred between 10:30am and 1:45pm. Thank you for your cooperation today. Please email me at: lisa.hudson@doe.virginia.gov with any questions.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation of the center and playground, equipment was not maintained in a safe and operable condition.
Evidence:
1. The train play equipment located on the Toddler playground had a wooden wheel that has splintered and cracked.
2. Some of the mats used for napping in the Trainers class were cracked exposing the inside foam cushion which interferes with proper weekly sanitizing.

Plan of Correction: New mats were already ordered to replace the ones that are no longer in good repair.
Maintenance will either fix or replace the wooden wheel on the train.

Standard #: 8VAC20-780-440-B
Description: Based on observation of the center, mats were not identified for use by a specific child.
Evidence:
Staff were observed laying mats out for nap time in the Trainers Class. Not all mats were numbered to identify a specific child's use.

Plan of Correction: All mats are numbered to identify a specific child. Some of the numbers had rubbed off. Staff reminded to ensure that numbers to assign mats remain on the mats or are replaced if missing.
