An unannounced monitoring inspection was conducted today between the hours 10:45am-12:30pm. There were 53 children directly supervised by 10 staff. The physical plant, 4 staff records, 5 children?s records, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed playing outside, playing with blocks, using a sensory bin, and working on crafts to include a year-long alphabet book project. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-140-B Description: Based on record review, the physical examination for one child obtained by the center did not occur within the required time period.
Evidence:
1. Child #1 was 3 years 5 months old on their first date of attendance (1/18/2023).
2. The date of examination provided on the physical was "8/18/2017".
3. The date provided on the physical occurred more than 12 months prior to attendance, outside of the time period required for Child #1's age.
4. The date provided on the physical occurred prior to Child #1's date of birth.Plan of Correction: We will request a new physical.
