An unannounced monitoring inspection was conducted on 05/24/2023 from 11:20 a.m. to 12:45 p.m. There were 20 children present, ranging in ages from two to four years old with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and three staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Based on review of records and interview, the center failed to ensure that each child?s record contained the required information.
Evidence:
1. Child #1, #2, and #3 ?s files were missing the work phone number of one parent.
2. Child #5?s file was missing the work phone number for both parents, place of employment of both parent and the address for the emergency contacts.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-340-F Description: Based on observation and interview, the center failed to ensure that children under the age of 10 years old are always within actual sight and sound of supervising staff.
Evidence:
1. In the large classroom, Staff #1 was observed to be alone with five children who were watching television in the front corner of the room.
2. During the inspection, Staff #1 went into the hallway twice and into the restroom twice leaving the children who were watching television out of sight.

Based on review of records, observation and interview, the center failed to ensure that at least one staff in each classroom or area where children are present had cardiopulmonary resuscitation (CPR) certification.

1. Staff #1 was observed to be the only staff present with the large classroom where five children were present.
2. Staff #1?s CPR certification had expired on 05/13/2023.
