Provider was reminded to follow-up on VA and out-of-state CPS checks for new staff member. Reminder was given to ensure that all children are properly strapped in when using the feeding tables. Provider was also reminded to obtain TB results for new staff members by their first day and that the TB results must be dated within 30 days prior to the start date.

An unannounced Monitoring Inspection took place on 05/24/2023 between the hours of approximately 8:55 a.m. and 10:45 a.m. There were 3 classrooms observed with a total of 35 children with 7 staff. The children were observed eating breakfast, participating in circle time and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-350-B-4 Description: Based on observation, the staff-to-children ratios were not always maintained whenever children were in care.
