An unannounced monitoring inspection was conducted on 05/31/2023 from 11:30am to 12:55pm. There were 43 children ages 3 months to age 5 years old supervised by 8 staff. The physical plant, outdoor playground area, programming, attendance, menus, 3 staff records, 3 children?s records, 1 medication with corresponding authorization records, emergency drills, injury reports, emergency supplies, and policies were reviewed. Children were observed participating in nap, table toys, reading and lunch. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 6/6/23. Please email me at angela.dudek@doe.virginia.gov with any questions.

Violations:
Standard #: 8VAC20-780-80-A
Description: Based on review of the written attendance record and interview with staff, the center did not ensure they maintained a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.

Evidence: During inspection in the Twos classroom, there were 10 children present, but the written record reflected there were 9 children present.

Plan of Correction: Staff will retrain in the Attendance Procedures.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center did not ensure that the surface for diapering was non-absorbent.

Evidence: In the Toddler classroom, the diapering pad was torn at the corner causing the surface to not be non-absorbent as required.

Plan of Correction: A new changing pad was requested during the inspection. It was received by maintenance by 4:00pm 5.31.23.
