Comments:
A renewal inspection was initiated on 6/5/2023 and concluded on 6/5/2023. There were 67 children directly supervised by 11 staff members. Children were observed during engaged in group activities, science, color project, rain coloring project and manipulatives. The physical plant, 3 child records, 6 staff records, medications, evacuation drills, and playground were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-70
Description: Based on a review of staff records, staff records did not contain two references before employment.
Evidence: Staff #1, date of employment 4/20/2023 and staff # 3, date of employment 3/13/2023 does not have documentation of 2 or more references on file. Both staff were observed caring for children.

Plan of Correction: References will be obtained and placed in the staff's record.

Standard #: 8VAC20-780-240-C
Description: Based on a revie of staff records, staff have not completed all the requirements of orientation training prior to working alone with children and no later then 7 days of the day of assuming job responsibility.
Evidence: Staff # 1, date of employment 4/20/2023 and staff # 2, date of employment 5/16/2023 have not taken the training in recognizing child abuse and neglect. Both staff were observed caring for children.

Plan of Correction: Staff will take the required training and file the certificate in the staff's record.
All staff will take training within 7 days of employment.

Standard #: 8VAC20-780-350-B-2
Description: Based on observation and children record review, staff to child ratio is not maintained in every classroom.
Evidence: The toddler classroom, ages 16 months- 24 months requires a 1 staff to 5 children. The classroom was observed with 2 staff and 10 children. One child, (child #1) was younger than 16 months, needing a ratio of 1 staff to every 4 children. The child is 12 months, the staff to child ratio was not being met for the youngest child in the classroom

Plan of Correction: All classrooms will maintain required ratios. Management will implement a plan when staff call out of work to always maintain ratios.

Standard #: 8VAC20-780-440-B
Description: Based on observation, cribs were not identified for use for each specific child.
Evidence: There were 3 infants observed sleeping in cribs. These 2 cribs were not labeled with each of the child's name.

Plan of Correction: All cribs will be labeled today.
