Comments:
An unannounced monitoring inspection was conducted from 11:45 am - 2:00pm with the center director. There were 38 children, ranging from three months to five years of age, present with seven staff supervising. Infant care was observed and older children were observed preparing to nap and napping. Four child records and three staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, five medications and authorization forms, ratios and required posted information.

Standard #: 8VAC20-780-70
Description: Based on review of three staff records, not all records contained all required information. Evidence: the record for staff #3 (date of hire: 5/8/23) did not contain documentation that two or more references were checked before employment.

Standard #: 8VAC20-780-330-B
Description: Based on observation, there was not resilient surfacing in the fall zone under and around the swings as required. Evidence: there was approximately nine feet of resilient surfacing behind the swings. The height of the swing beam was approximately 6 feet high which requires a fall zone with resilient surfacing of 12 feet (measured from the swing itself) in both the front and back of the swings.

Standard #: 8VAC20-780-560-J
Description: Based on observation, and an interview with staff, not all tables were sanitized, as required, after meals. Evidence: staff used disinfectant wipes to sanitize the tables after lunch in the 3?s classroom. "Sanitized" means to spray the disinfectant solution on the surface and allow it to air dry for a minimum of two minutes.
