Comments:
An unannounced monitoring inspection was conducted on Monday, May 22, 2023 to determine the center's compliance with licensing standards. The inspector was on site from 9:35am to approximately 1:00pm. There were a total of 21 children in care in the direct care of four staff members. The center's director assisted the inspector throughout the inspection. Upon the inspector's arrival, the children were observed cleaning up after eating breakfast. The children and staff were later engaged in various activities. The children were observed during transitions, during free-play, and teacher-led activities. The center is equipped with age appropriate materials and equipment for the children's use. Staff were engaged with the children and offered guidance when needed. The areas where children receive care were inspected and found to be in compliance. The required postings were observed. Per the director, medication is not administered and transportation is not provided at this time. During the inspection, five children's records and five staff records were reviewed.

Information gathered during the inspections determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-245-A
Description: Based on a review of five staff records and interview, the center did not ensure one staff completed annually a minimum of 16 hours of training appropriate to the age of children in care.

Evidence: 1) The record of Staff #5, employed on 04/12/22, contained a total of 12 hours of annual training from 04/2022 - 04/2023.

2) During interview, a member of management confirmed the staff member was missing 4 hours of annual training.

Plan of Correction: Staff #5 will complete the 4 hours of training that are missing. In the future, staff will complete 16 hours of annual training.

Standard #: 8VAC20-780-340-D
Description: Based on a review of staff records and staff interviews, the center did not ensure at least one staff member who meets the qualifications of a program leader or program director shall be regularly present in each grouping of children.

Evidence: 1) Staff #3, employed on 12/03/2022, was observed supervising six toddler/two year old children during the inspection. Staff #3 was the only staff member supervising this grouping of children.

2) A review of Staff #3's record showed the staff member does not meet program leader qualifications.

3) During interview, a member of management confirmed Staff #3 is regularly in a grouping of children by herself and verified the staff member does not meet program leader qualifications.

Plan of Correction: Staff #3 is in the process of completing the training needed to complete the required hours of training to meet program leader qualifications. The staff will not be unsupervised until she is able to have the required time of supervised programmatic experience.

Standard #: 8VAC20-780-540-C
Description: Based on observation and interview, the center did not ensure the first aid kit included the required supplies.

Evidence: 1) The center's first aid kit did not contain a minimum of two triangular bandages.

2) During interview, a member of management confirmed there were no triangular bandages in the first aid kit or available at the center at the time of the inspection.

The required first aid kits shall include a minimum: scissors; tweezers; gauze pads; adhesive tape; band-aids, assorted types; an antiseptic cleansing solution/pads; thermometer; triangular bandages; single use gloves; and the first aid instructional manual.

Plan of Correction: The center ordered the triangular bandages and will ensure they always have at a minimum two triangular bandages in the required first aid kits.
