Comments:

A monitoring inspection was conducted on 6/02/2023 with the center director and staff. There were 36 children present, ranging in ages from 5 months to 12 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed. The children were waking from rest, having afternoon snack, and playing toys. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	8VAC20-780-140-A
Description:	Based on review of five children's records, the facility failed to have a physical examination for each child within 30 days of attendance. Evidence: Child 1 (date of enrollment 1/20/2023) and Child 5 (date of enrollment 1/28/2021) did not have documentation of a physical in the file.
Plan of Correction:	The information will be requested from the parent.
Standard #:	8VAC20-780-160-A
Description:	Based on review of three staff records, the center failed to obtain documentation of a negative tuberculosis (TB) screening or test at the time of employment and prior to coming in contact with children. Evidence: Staff 2 (date of employment 5/1/2023) did not have documentation of a TB test on file.
Plan of Correction:	TB tests will be collected from each staff prior to employment.

Description:	Based on review of three staff records, the facility failed to have required documentation in each file. Two references as to character, reputation, and competence should be checked prior to employment. Evidence: Staff 2 (date of employment 5/1/2023) did not have documentation of two references on file.
Plan of Correction:	References will be checked and documented prior to employment.

Description:	Based on review of three staff records, the facility failed to have documentation of orientation for staff prior to working alone with children. Evidence: Staff 1 (date of employment 5/09/2023) and Staff 3 (date of employment 5/08/2023) did not have documentation of orientation on file.
Plan of Correction:	Staff will receive orientation in the required time frame.
Standard #:	8VAC20-780-350-C
Description:	Based on observation, the facility failed to maintain the required ratio of the youngest child in the group when the children are in mixed age groups. The required ratio for is one staff for every 4 children ages birth to 16 months. Evidence: Three staff were alone with 14 children ages 5 months to 5 years from approximately 2:30 p.m. to 3:00 p.m.
Plan of Correction:	Ratios were reviewed with center director. Ratios will be maintained.
