Comments:
A complaint was received by the Office of Child Care Health and Safety on 5/22/2023 in the areas of supervision and staffing. An unannounced complaint inspection was conducted on 5/23/2023. There were 95 children in care during the inspection. There were 9 staff present upon
arriving to the center and 7 additional staff arrived during the inspection.


The information gathered during the inspection did support the complaint and therefore the
complaint was determined to be valid. Information gathered during the course of the investigation
determined non-compliance with applicable standards or law. Violations both related and unrelated the complaint were documented on the violation notice issued to the provider. The inspector arrived at 8:05 AM and departed at 12:30PM.

Based on observation, the center failed to maintain the required staff-to-children ratio for birth to 3 year to up to school age eligible.

Evidence:
At approximately 8:15 am, 30 preschool children were in care under the supervision of two staff on the outdoor playground area. The youngest child was 3 years old. The ratio is one staff to 11 children with an allowable variance for ratio. Three staff would be required.

Plan of Correction: Preschool teacher clocked in right at 8:15 and came out to the playground. We have adjusted to 10-hour days and other schedules to ensure we have ratio coverage

Description: Based on observation, the center failed to ensure there shall be at least 12 inches of space between occupied cots, beds, and rest mats.

Evidence: Observed 15 children resting on cots. There were multiple children on cots that were directly touching other children?s cots.

Plan of Correction: Teachers have been reminded of the 12inches for mats or cots when children are sleeping.
