A self-report was received by the Office of Child Care Health and Safety on 5/23/2023. An
unannounced inspection was conducted 5/30/2023. The inspector was on site from 4:10pm to 5:15pm.
The information gathered during the inspection did not support the self-report and therefore the self-report was determined to be invalid. Information gathered during the course of the investigation determined non-compliance with applicable standards or law. Violations unrelated to the complaint were documented on the violation notice issued to the provider.

Standard #: 22.1-289.035-A Description: Based on record review, the center failed to renew the Central Registry check after expiration of 5 years.
Evidence: Staff #1 had a Central Registry on file dated for 5/2/2018. A new Central Registry check not available during the inspection on 5/23/2023.
Staff #2 had a Central Registry on file dated for 5/9/2018. A new Central Registry check was not available during the inspection on 5/23/2023.Plan of Correction: Fingerprints were scheduled by the end of the month.
