An investigation was conducted on 5/2/2023, in response to a self-reported concern received by the Licensing Office regarding supervision. Interviews were conducted, written information and attendance records were reviewed. The information gathered during the investigation determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-40-E Description: Based on a review of policies and procedures, interviews and a review of documentation, the center did not ensure that the center's activities and services were in compliance with the center's own policies and procedures related to supervision that are required by the standards.
Evidence:
1. Staff #1 and Staff #2 transitioned a group of children from the playground to the main building on 3/30/2023. During the transition, a two-year old Child #1 was left behind on the playground.
2. Threshold counts and name-to-face checks were not completed when they were exiting the playground.
3) The center's Child Supervision Policy requires that children must be under adult supervision by sight and sound at all times. The center's policies require that "Name-to-fac checks or threshold counts must be conducted and documented every time the children move from one location to another, such as when the children move from the classroom to the gym, from the classroom to the playground and from the playground to the classroom".Plan of Correction: Retrain staff on threshold counts and name-to-face.
Standard #: 8VAC20-780-340-F Description: Based on interview and review of documentation, a child was not within actual sight and sound supervision of staff.
Evidence:
1. On 3/30/2023, at approximately 10:30am, Staff #1 and Staff #2 were on the playground with the Toddler 2 classroom.
2. Upon returning to the main building as the children and staff were entering through the main building doors, Staff #1 and Staff #2 realized that they were missing a child.
3. Staff #1 immediately ran back to the playground and found Child #1 sitting by the storage shed.
4. Based on the distance from the playground to the main building, the child was not within actual sight and sound supervision for 1 to 2 minutes.Plan of Correction: Retrain staff on threshold counts and name-to-face.
