Comments:

An unannounced monitoring inspection was conducted with the director on May 11, 2023, beginning at 10:49am and ending at 1:13pm. There were 27 children, ages 6 months through 5 years old, in the care of 7 direct care staff. The children were observed playing with age-appropriate toys and playing on the playground. Five children?s records were reviewed. Five staff records were reviewed. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. One medication was reviewed and in compliance. The daily attendance and emergency drill log were reviewed. The most recent evacuation drill was documented as 3/30/2023. The most recent shelter-in-place drill was documented as 2/15/2023. The most recent health inspection was dated 3/17/2023. The most recent fire inspection was dated 12/01/2022. Description:	Based on review of documentation, the facility did not obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. Evidence: Staff 1 and staff 2 did not have documentation of out of state background checks in their record.
