Comments:
An unannounced monitoring inspection was conducted on 6/7/2023 from 8:15 am-10:15 am. There were 56 children supervised by 13 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, policies, and injury reports were inspected. Children were observed participating in group play and tabletop activities. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22.1-289.035-B-4
Description: Based on record review and interview, the center failed to obtain the results of an out-of-state central registry check for all states employees have resided in within the previous five years.

Evidence: There was no documentation of an out-of-state central registry check for one of two states for Staff #4 (DOH: 7/8/2019), who indicated that they had lived outside of Virginia within the past five years on their sworn statement form dated 07/05/2019.

Standard #: 8VAC20-780-60-A-8
Description: Based on medication records and interview, the center failed obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: There was no documentation of a written care plan for child #5 who has a diagnosed food allergy.
