An unannounced monitoring inspection was conducted today between the hours of 4:40pm and 5:15pm. There were 11 children directly supervised by 3 staff. The physical plant, 2 staff records, 3 children?s records, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed participating in group play, reading books, playing outside, coloring, and completing crafts. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-780-60-A Description: Based on record review, 3 of 3 children's records reviewed did not contain all required information.
Evidence:
1. The record for Child #1 did not contain the work phone number and place of employment of each parent who has custody. The record for Child #3 did not contain the work phone number for the place of employment for 1 parent.
2. The record for Child #2 did not contain the name, address, and phone number of one of two designated people to call in an emergency if a parent cannot be reached. The child's father was listed as the second emergency contact person.Plan of Correction: I will get the information from the parents by Friday.
