Comments:

An unannounced focused monitoring inspection was conducted on June 5, 2023 from approximately 10:45 am to 12:00 pm. There were 25 children in attendance and a total of 6 staff present. 5 staff records and 4 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed taking a nap and playing with table toys while the licensing inspector was on-site. Violations were cited as a result of this inspection. Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 06/14/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Description:	Based on records review, the center failed to have a Central Registry finding within 30 days of employment. Evidence: The record for staff #5, date of employment 08/21/2022, did not contain the results of a Central Registry search.
Plan of Correction:	Per the center "waiting on response from DSS."
Standard #:	8VAC20-780-160-A
Description:	Based on records review, and observation, the center failed to ensure that staff submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. Evidence: The record for staff #4, date of employment 05/26/20, did not contain documentation of a negative tuberculosis screening. Staff #4 was observed working in the classroom with children.
Plan of Correction:	Staff obtained a negative TB test on June 8th 2023.

Description:	Based on records review, and observation, the center failed to document that two or more references as to character and reputation as well as competency were checked before employment. Evidence: The record for staff #5, date of employment 08/21/2022, did not contain documentation of references. Staff #5 was observed working in the classroom with children.

Description:	Based on records review, the center failed to ensure that staff complete the Virginia Department of Education (VDOE) sponsored orientation course within 90 calendar days of employment. Evidence: The record for staff #4, date of employment 05/26/20, and the record for staff #5, date of employment 08/21/2022, did not contain documentation of the VDOE orientation course being completed.

Description:	Based on records review, and observation, the center failed to ensure that staff complete orientation training no later than seven days of the date of assuming job responsibilities. Evidence: The record for staff #5, date of employment 08/21/2022, did not contain documentation of completion of orientation. Staff # 5 was observed working in the classroom with children.

Description:	Based on observation, the center failed to ensure that cots used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover. Evidence: There were 7 cots occupied by sleeping children that did not have a bottom cover.

Description:	Based on documentation review, and interview, the center failed to maintain documentation of emergency evacuation drills. Evidence: There was no drill documented for May of 2023. The Assistant Director acknowledged that the drill was not documented.
Plan of Correction:	Per the center "drills were done just not documented. We have documented them and will continue to do so."
