Comments:
An unannounced complaint inspection was initiated by a local agency representative and inspector on May 19, 2023 and completed on June 13, 2023 regarding concerns of staffing and supervision and programs. There were 3 children present with 1 staff supervising. There were observations of a parent drop off and local school bus pick up of the children at the center. During the inspection the following were completed: interviews with administrative staff, a review of records/documentation, observation of the classroom in which the incident occurred, and a review of video footage of the incident on May 15, 2023. Additionally, there were staff and collateral interviews, and a review of pertinent documentation during the investigative process. The preponderance of the evidence gathered during the investigation does support the allegation related to staffing and supervision; therefore, the complaint is determined to be valid. The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: 1. A review of video footage of an incident on May 15, 2023 in the four year old KPA classroom revealed that child #1 was sitting on top of a shelf, approximately 2.5 feet high, while interacting with other children. A child was observed to move behind child #1 and push child #1 off the shelf and onto the floor. Child #1 was observed to land directly on their face.

2. Interview with Staff#1 revealed Child#1 was observed sitting on top of a shelf, approximately 2.5 feet high while Staff#1 was assisting other children at the sink in the classroom. When staff #1 noticed child #1 sitting on top of the shelf, staff #1 did not intervene to have the child move to a space that was more appropriate in order to prevent a possible fall or injury. Staff#1 stated her back was turned when she heard "a loud boom" and noticed Child#1 on the floor. Staff#1 reported Child#1 sustained a knot to the child's forehead with a slight scrape. The child's forehead was cleaned with a wet paper towel, and the incident was reported to the supervisor in charge at the center.

3. Interview with collateral confirmed that there was an incident on May 15, 2023 reported by the center. Collateral also stated that Child#1 was picked up from the center with noticeable injury to child's forehead and eye with significant swelling. Child#1 was reported to be personally transported to the emergency department as a result of the injuries and existing condition.

4. A review of medical documentation from May 15, 2023 indicated a diagnoses of minor head injury and traumatic hematoma of the forehead of child #1. On May 17, 2023 there was an additional diagnoses, that was observed in an additional medical report, of a head injury due to trauma and ecchymosis of the eye.

5. A review of the incident/accident report documented by the center confirmed the incident on May 15, 2023 involving Child#1 in the 4 year old KPA classroom. The report indicated that child #1 was sitting on top of the shelf and fell off the shelf which resulted in the child's head hitting the ground.

Plan of Correction: The center's director will ensure all existing and new hired staff complete indoor and outdoor supervision training and staff will sign documents for completion.
