A renewal inspection was conducted Tuesday, June 13, 2023. The licensing inspector was onsite, completing the inspection from approximately 10:10am until 1:00pm. There were 32 children present, ranging in ages from two years to nine years, with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and six staff members? records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-C Description: Based on review of six staff records, observation and documentation review, the center did not ensure one staff member obtained and submitted documentation of a follow-up tuberculosis (TB) screening at least every two years from the date of the initial screening.
Evidence:
1. The record for Staff #5 (Date of Employment 01/11/2021) contained a TB screening dated 01/12/2021.
2. Staff #5 was observed working and counting in the staff-to-child ratio during the inspection.
3. Staff #1 confirmed this information during the 06/13/2023 inspection.Plan of Correction: The will have the staff to schedule an appointment for an updated TB screening.
Standard #: 8VAC20-780-270-A Description: Based on observations and interview, the center did not maintain all areas and equipment in a clean, safe, and operable condition.
Evidence:
1. The vent located on the wall in the Wise Owls? classroom was dirty with dust build up.
2. The walls in the Wise Owls? and Dr. Suess? classrooms had areas of peeling paint.
3. Children were observed being cared for in the Wise Owls? and Dr. Suess? classrooms during the inspection.Plan of Correction: The landlord has already been made aware of the situation and painting and cleaning of the vents will take place.
Standard #: 8VAC20-780-560-F Description: Based on observation and interview, the center did not ensure a menu listing food to be served for meals and snacks during the current one-week period was posted in a location conspicuous to parents or given to parents.
Evidence:
1. A menu was posted in the hallway contained the food items being served for 06/12/2023 and 06/13/2023.
2. Staff #1 stated the menu is created one day at a time based on the children?s likes and dislikes of food items.Plan of Correction: I will type and post a weekly menu. Any substitutions will be indicated on the posted menu
Standard #: 8VAC20-780-560-F-4 Description: Based on observation of the lunch food served to children, the center did not ensure that children under three years in age were not served foods that are known choking hazards. Evidence:
The children in the Wise Owls? classroom (two years of age) were served whole green grapes at lunchtime.Plan of Correction: The staff were aware that the grapes needed to be cut into smaller pieces. I will remind the staff again.
