Comments:
A monitoring inspection was initiated on 06/13/2023 and concluded on 06/13/2023 from 12:50 PM to 2:40 PM. There were 27 children present, ranging in ages from 5 years to 12 years, with nine staff supervising the class and a second staff member available if needed. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of five children?s records and the five staff?s records were reviewed for updates.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 8VAC20-780-70
Description: Based on record review and interview, the center failed to obtain all required information for one out of five staff records reviewed.

Evidence:

1. The record for staff 1 only contained one reference. At least two references are required before employment. The hire date for staff 1 was documented as 6/7/23 and the start date was documented as 6/12/23.

Plan of Correction: In the future the director will be responsible for ensuring at lease two references are completed and documented prior to employment for each new staff member.
