Comments:
A monitoring inspection was conducted on 6/14/2023 with the licensing inspector being on site from approximately 2:25pm to 4:30pm. The center director and center staff were available throughout the inspection to answer any questions. There were 23 children present with 5 staff
supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing, supervision, programming, injury reporting, medication, special care and emergencies and nutrition. Medication is administered. During the inspection, the children and staff were observed participating in a variety of activities. Interviews were conducted with staff and areas of the facility were inspected to include all the center's bus, classrooms, bathrooms and
the playground areas. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-330-B
Description: Based on observations and measurements, the center did not ensure that playground equipment had resilient surfacing that complies with the minimum safety standards when
tested in accordance with the procedures described in the American Society for Testing and Materials.
Evidence:
1. The slides and climbing equipment located on the center?s playground did not have enough resilient surfacing throughout the fall zones. Fall zones are defined as the areas underneath and surrounding equipment that requires a resilient surface.
2. The center had approximately 2 to 4 inches of wooden mulch, decomposed mulch and dirt
throughout the fall zones on the preschool ground.
3. The equipment required a minimum of 6 inches of resilient material underneath, around
the slides and in the fall zones.

Plan of Correction: I will get mulch ordered ASAP.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center did not ensure a nonabsorbent surface for diapering or changing shall be used.
Evidence:
The two year old classroom and the Creepers/Toddler classroom had several tears on their diapering surfaces making the surface absorbent.

Plan of Correction: Will order pads today.
