Comments:

A complaint inspection was initiated on 5/8/2023 and concluded on 6/14/2023. There were 19 children present with five staff supervising. The inspector reviewed compliance in the areas of programming, special care, and background checks . A total of seven staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and date to be corrected? for each violation cited on the violation notice and return it to me within five (5) business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring gain; and 3) person or person(s) responsible for implementation.

Description:	Based on observations the center failed to ensure that all the required documents are posted in areas accessible to the public. Evidence: The Notice of Intent dated 1/30/2023 was not posted on 5/8/23.

Description:	Based on record reviews the center failed to ensure that central registry checks are submitted within seven calendar days of the person being employed and received within 30 days of employment. Evidence: Staff 5 (hire date 5/1/23), staff #6 (hire date 5/8/23), staff #4 (hire date 4/24/23), staff #3 (hire date 4/19/23), and staff # 1 (hire date 4/27/23) had their central registry request submitted 5/21/23.
Plan of Correction:	Ensure updates on requests added to staff files, requests were submitted within 7 days the 5/21/23 was resubmission due to changes in central registry. [sic]

Description:	Based on record reviews the center failed to ensure that staff shall complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. Evidence: Staff #3 (hire date 4/19/23), Staff #4 (hire date 4/24/23), Staff #5 (hire date 5/1/23) and staff # 6 (hire date 5/8/23) do not have documentation on file of orientation training being conducted.

Description:	Based on observation the center failed to ensure that pillows and filled comforters shall not be used by children under two years of age. Evidence: An infant in the crib was on a boppy pillow.
Plan of Correction:	Staff has been reminded that boppy pillows can not be used in cribs. [sic]

Description:	Based on observation the center failed to ensure that bottle fed infants who cannot hold their own bottles shall be held when fed. Bottles shall not be propped or used while the child is in his designated sleeping location. Evidence: Upon entering the infant area on 5/25/23 at 12:30 pm two infants, one in a crib the second in a bouncy chair had their bottles propped with blankets. There were four infants with two staff in the infant area during this time.
Plan of Correction:	Staff has been reminded that is unacceptable behavior. All infant staff members have been assigned retraining on infant feeding and safe sleep practices. 6/25/2023 for completing trainings [sic]
