Comments:
A monitoring inspection was initiated and concluded on 6/9/2023. There were 40 children present with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. The inspection started at 11:00 am and concluded at 2:30 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-510-P
Description: Based on a review of medications and authorization forms stored on site, the licensee did not ensure that when an authorization for medication expires, the parent shall is notified that the medication needs to be picked up within 14 days or the parent must renew the authorization and that medications that are not picked up by the parent within 14 days are safely disposed of by the center.
Evidence:
An EpiPen, stored on site for emergency use was authorized by a signed form which expired 3/19/2023. The medication had not been returned to the parent or disposed of within 14 days of the authorization form expiring.

Plan of Correction: The parent will be advised to have a new authorization form completed and submit it to the center by Monday or pick up the medication from the center at close of business Monday until a new authorization form is submitted.
