Comments:
An unannounced monitoring inspection was conducted on 6/1/23 from 10:30 a.m. ? 12:30 p.m. There were fifty-four children present with ten staff providing supervision. The ages of the children ranged from 3 months to 5 years. Four staff files and four children?s files were reviewed. Children were observed playing with slime and engaging in water play. Handwashing was observed. Attendance, menus, and emergency drills were reviewed. There was an adequate number of staff present with certification in Medication Administration Training (MAT), CPR, First Aid and Daily Health Observation (DHO) training. Three medications and their authorization forms were reviewed. Required postings were observed. The last emergency drill was conducted on 5/23/23. The first aid kit, flashlight, and battery operated radio were observed. Areas of non-compliance are identified in the Violation Notice.

Description: Based on a review of four staff records, the center did not obtain documentation of the results of a central registry search within 30 days of employment for all staff.

Description: Based on observation, it was determined that not all areas of the center inside and outside were maintained in a clean, safe condition.

Evidence: 1. In the Two?s bathroom there was one screw that extends approximately 6
thread lengths at the base of the toilet that can create an impalement and
scratch hazard.
2. In the Two?s classroom a protective plastic strip was removed from the cubbies exposing splintering wood.
3. On the Pre-K/School Age playground there was one screw that extends approximately 11 thread lengths on a chain link enclosure that stores the facilities gas pump.
4.The resilient surface material, on the toddler playground, has formed a bump in the play area, creating a trip hazard.

Evidence: The toddler class was observed playing in a sprinkler/wading pool at the time of the inspection. None of the toddler?s were potty trained.

Description: Based on observation and interview, children?s hands were not always washed with soap and running water after toileting.

Evidence: In the Preschool I class a wipe was used to clean a child?s hands after diapering. Staff confirmed that soap and running water is not used after diapering the child, that wipes are used instead.
