As discussed, please forward any plans for the anticipated new playground layout/equipment for review prior to installation.

Comments:
An unannounced Monitoring inspection was conducted today. A total of 89 children with 18 staff were onsite. Observation of all 8 classrooms were made. In most classrooms, the children had finished lunch and were transitioning to nap time. Staff were observed sitting in between children while they napped to ensure adequate supervision. The center had just completed a full evacuation drill the day before and were pleased with the protocols and process time it took to evacuation off site. In one classroom, A Sleeping Beauty Show was being planned which included costumes for the children. Staff were aware of specific needs of the children pertaining to food allergies and any other medication needs. Handwashing and diapering procedures were observed and found in compliance. Each classroom had stocked supplies and equipment available to ensure the current lesson planning was carried out. Only 2 new staff hired since the last inspection. A total of 4 staff records were reviewed and random background checks, TB screenings and certifications for other staff were reviewed also. Recordation system for staff was found very organized and tracking systems have been developed to ensure the files remain current. A total of 7 children's records were also reviewed. Standard #: 8VAC20-780-130-E
Description: Based on child record review, additional immunizations were not obtained every six months for 2 children under the age of 2 years. .
Evidence:
Child #4 is under the age of 2 years. Last documented immunizations were dated in October 2022. Child #6 is under the age of 2 years. Last documented immunizations were dated in July 2022.

Plan of Correction: The office administrator will print out a monthly report to audit the immunization records. They will notify parents who have immuniztions coming due.

Standard #: 8VAC20-780-80-A
Description: Based on observation of one classroom, the written record of daily attendance did not document the arrival of one child as it occurred.
Evidence:
There were a total of 7 children in attendance in Room 1A. The record of daily attendance maintained in the classroom documented the arrival for only 6 children .

Plan of Correction: Teachers will add the time a child arrived if a parent forgets to sign the child in and will highlight the time the input so parents are aware did not sign their child into the classroom.

Standard #: 8VAC20-780-510-G
Description: Based on review of medications onsite, one medication was not properly labeled.
Evidence:
An emergency medication found in Room 1B was not labeled with the child's name, dosage amount and time/times to be given.

Plan of Correction: Classrooms will utilize Medication Intake Log when receiving medication from parents as well as conduct a monthly inspection of on-site medications using the Medications and Authorizations Inspection Log.

Standard #: 8VAC20-780-520-A
Description: Based on observation of the topical ointments in room 2C, two over the counter skin products were kept beyond the expiration date.
Evidence:
Eucerin was found in Room 2C which expired in October 2021. Aquaphor was found in Room 2C which expired in May 2023.

Plan of Correction: Classrooms will utilize Medication Intake Log when receiving medication from parents as well as conduct a monthly inspection of on-site medications using the Medications and Authorizations Inspection Log.
