An unannounced inspection was initiated on 4/20/2023 and completed on 5/26/2023 in response to a complaint that was received by the licensing department on 4/11/2023 and 5/9/2023 regarding allegations of staffing and supervision. On 4/20/2023, the inspector was on site from approximately 9:20AM until 11:30AM. Interviews with staff were completed, observations were made in the center, video footage was reviewed, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be not valid. See the violation notice for more details.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Evidence:
1. The injury reports did not contain signatures of a staff and parent or two staff signatures.
2. Injury report did not document any future action to prevent recurrence of the injury.
