Comments:
Today an unannounced renewal Inspection took place between the hours of 1:35pm and 3:50pm.There were 7 classrooms observed with a total of 55 children with 9 staff within the supervision guidelines. The children were observed receiving bottles and napping. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard #: 22.1-289.011-F
Description: Based on observation, the findings of the most recent inspection of the facility was not posted in a conspicuous place on the licensed premises.
Evidence: The most recent inspection findings were not posted.

Plan of Correction: We will get the new inspection findings posted once received.

Standard #: 8VAC20-780-70
Description: Based on record review and interview, staff records did not contain all required information.
Evidence:
1. Staff #8's record does not contain documentation of education.
2. Staff #9's record only contains one professional reference.
3. Staff #10's record only contains one professional reference.
4. Staff #11's record only contains one professional reference.

Plan of Correction: I need to get a few additional professional references and I'll follow up with them. I will request another copy of the diploma or CDA.

Standard #: 8VAC20-780-270-C
Description: Based on observation inside areas occupied by children, the temperature was not maintained above than 68?F.
Evidence: The thermostat in the Radici classroom was set at approximately 58?F. Infants were observed in the classroom during the inspection.

Plan of Correction: Temperature was corrected during the inspection. I will send out a reminder to staff that the temperature shouldn't be below 68 degrees.

Standard #: 8VAC20-780-280-D
Description: Based on observation, cleaning and sanitizing materials were stored next to food equipment, utensils and/or single-serve articles.
Evidence: Bleach cleaners were observed stored on the same shelf next to food equipment in a cabinet in the Gambo classroom.

Plan of Correction: I'll have the cups and silverware moved up to the next shelf.

Standard #: 8VAC20-780-490-E
Description: Based on observation, surfaces that become contaminated with body fluids cannot be cleaned and sanitized.
Evidence: There were tears observed on the surface of the diaper changing mat in the Gambo classroom. The inner foam pad of the mat cannot be cleaned and sanitized.

Plan of Correction: I'll replace the mat.

Standard #: 8VAC20-780-570-E
Description: Based on observation, infant bottles were not dated with todays date.
Evidence: Two bottles in the infant classroom were observed with the date labeled on the bottles.

Plan of Correction: We ask the parents to date the bottles, but if they don't do it, the staff should be doing it. I'll remind the staff.
