An unannounced monitoring inspection was conducted on May 22, 2023 in response to a self-reported incident that was received on May 17, 2023 in reference to a child injury. Interviews were conducted and documentation was reviewed. The preponderance of evidence gathered during the investigation does not support violation of standards regarding the self-report.
No violations were cited.
