A Subsidy Health and Safety inspection was conducted on 06/20/2023 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 8VAC20-790-540-B-3 Description: Based on record review, 2 of 4 children?s records reviewed did not include all required information.
Evidence:
1. The records for Child #1 and Child #3 did not contain the address for each custodial parent?s place of employment.Plan of Correction: The center will follow up with the parents to obtain missing information.
Standard #: 8VAC20-790-600-D-1 Description: Based on record review, 2 of 3 staff did not obtain current certification in cardiopulmonary resuscitation and current certification in first aid within 90 days of the date of employment.
Evidence:
The record for Staff #2 (date of employment: 7/19/2022) did not contain documentation of CPR and First Aid certification.
The record for Staff #3 (date of employment: 7/1/2022) did not contain documentation of CPR and First Aid certification.Plan of Correction: The center will check their email to obtain documentation of certifications for staff. Staff that do not have current certifications will be scheduled.
